Introduction
Guidelines:
- medications in liquid form are generally preferable
- sublingual, rectal, transdermal & IV are alternative routes
- some (not all) IV medications can be given orally or through a feeding tube
- some capsules may be opened & intact pellets can be administered through large-bore feeding tubes (see below)[1]
Capsules that can be opened for administration of intact granules:
- sustained-release medications
- acetazolamide Diamox Sequels
- carbamazepine: Equetro, Carbatrol
- dexmethylphenidate: Focalin XR
- dextroamphetamine: Adderall XR
- diltiazem: Cardizem CD, Taztia XT, Tiazac
- duloxetine: Cymbalta
- erythromycin: ERYC
- esomeprazole: Nexium
- indomethacin: Indocin SR
- lansoprazole: Prevacid
- methylphenidate: Metadate CD, Ritalin LA
- morphine sulfate: Avinza, Kadian
- omeprazole: Prilosec
- potassium chloride: Micro-K
- theophylline: Theo-24
- verapamil: Verelan, Verelan PM
- enteric-coated medications
- budesonide: Entocort EC
- pancrelipase: Cotazym-S, Creon, Lipram, Pancrease MT, Pancrecarb MS, Pancrelipase
- other
Medications that should not be crushed:[1]
- sustained-release medications (except as noted)
- enteric-coated medications (except as noted)
- buccal tablets, sublingual tablets, & lozenges
- medications that can be irritating
- alendronate: Fosamax
- atomoxetine: Strattera
- benzonatate: Tessalon Perles
- diflunisal
- ganciclovir: Cytovene
- hydroxyurea: Hydrea
- ibandronate: Boniva
- isotretinoin: Accutane
- olanzapine: Zyprexa, Zydis
- piroxicam: Feldene
- risedronate: Actonel
- temozolomide: Temodar
- valproic acid: Depakene
- vorinostat: Zolinza
- medications that can be teratogenic
