Introduction
serum monoclonal IgM paraprotein > 3gm/dl
Epidemiology
- disease of old age
- presents in the 6th & 7th decades of life
Pathology
- bone marrow shows a diffuse infiltrate of lymphocytes & plasma cells comprising > 10% of bone marrow
- secretion of abnormal macroglobulin results in hyperviscosity syndrome
- can be seen in association with
- may resemble CLL more than multiple myeloma
- malignant cells derive from postgerminal B-cell with somatic hypermutation, but not switch recombination[6]
- impaired platelet function
- dysfibrinogenemia
Clinical manifestations
- chief complaints are generally non-specific
- weakness
- fatigability
- B symptoms of fever, night swears, & weight loss
- hyperviscosity syndrome is a medical emergency (31%)
- neurologic signs, neuropathy
- deafness
- excessive oozing from wounds
- ecchymosis
- lymphadenopathy[7]
- hepatosplenomegaly[7]
Laboratory
- serum & urine protein electrophoresis
- bone marrow biopsy & aspirate
- viscosity of serum if IgM > 4 g/dL or hyperviscosity syndrome suspected[7]
- negative findings
- serum complement levels are normal
- antinuclear antibody (ANA) is negative
- rheumatoid factor (RF) is negative
Diagnostic procedures
- ophthalmoscopy may reveal evidence of hyperviscosity
- dilated retinal veins (images[11])
- papilledema
- flame hemorrhages[7]
Radiology
- Chest X-ray
- no distinct radiologic lesions
- pleural effusion (50%)
- parenchymal infiltrates may be seen
Complications
- hyperviscosity syndrome is a medical emergency
- immediate plasmapheresis is indicated to remove excess IgM[7]
- hyperviscosity-related retinopathy[11]
Management
- plasmapheresis to remove IgM for hyperviscosity syndrome
- chemotherapy
- rituximab with or without cytotoxic chemotherapy[7]
- rituximab with nucleoside analogs with or without alkylating agents
- rituximab + thalidomide[9]
- cyclophosphamide-based therapy
- chlorambucil
- fludarabine
- 2-chlorodeoxyadenosine (cladribine)
- rituximab with or without cytotoxic chemotherapy[7]
- salvage therapy
- add bortezomib, alemtuzumab[9]
- bortezomib associated with peripheral neuropathy
- add bortezomib, alemtuzumab[9]
- stem cell therapy, autologous vs allogeneic[9]
- prognosis: average survival is 2-5 years
