Immunophenotype

• surface & cytoplasmic Ig, usually IgM +
• B-cell antigens (CD19, CD20, CD22, CD79a) +
• CD5 -
• CD10 -
• CD43 +/-

Genetics

Ig heavy & light chain genes rearranged

Clinical manifestations

• majority have Waldenstrom's macroglobulinemia
• hyperviscosity 10-30%
• neuropathies (paraprotein)
• cryoglobulinemia
• coagulopathy

Laboratory

• IgM paraprotein reactivity to myelin sheath antigens such as MAG or gangliosides ~10%
