Introduction
Excessive urination at night, including involuntary loss of urine during sleep.
Etiology
- nocturnal polyuria (> 35% of 24 hour urine output)
- late evening fluids, especially containing caffeine or alcohol
- peripheral edema
- venous stasis
- heart failure
- medications
- sleep disturbance
- medications
- cardiac disease or pulmonary disease
- pain
- depression
- restless legs syndrome
- obstructive sleep apnea
- sleep partner
- lower urinary tract disease
Laboratory
Diagnostic procedures
- polysomnography if
- sleep disordered breathing (suspected sleep apnea)
- unexplained or suspected pulmonary hypertension
Management
- treat benign prostatic hypertrophy in men
- desmopressin nasal spray (Noctiva) FDA-approved March 3, 2017
- not for patients with baseline serum sodium < 135 meq/L[1]
- referral to sleep specialist if associated with other sleep disorder[1]
