Introduction
Revised Criteria for Biological & Clinical Staging of Alzheimer Disease
Staging
- clinical: stages 1-6
- stage 1: cognitively nornal
- stage 2: subjective cognitive decline
- stage 3: mild cognitive impairment
- stage 4: mild dementia
- stage 5: moderate dementia
- stage 6: severe dementia
- biological: based on PET scan (T2 as a core 2 biomarker)
- includes 4 stages (A to D)
- all stages require beta-amyloid positive
- elevated tau-PET signal in the medial temporal lobe associated with high risk of progressing to MCI over the next few years
- stage A: beta amyloid +, tau -
- stage B: beta amyloid +, tau + in medial temporal lobe
- stage C: beta amyloid +, tau + in medial temporal lobe, moderate tau in neocortex
- stage D: beta amyloid +, tau + in medial temporal lobe, high tau in neocortex
* high tau-PET signal in the neocortex associate wit less clinical benefit of donanemab vs low to intermediate signal
Notes
- elderly with discordant clinical & biological stages of Alzheimer's disease generally have additional pathology[1]
