Etiology
(Differential diagnosis)
- subcorneal (beneath the stratum corneum) vesicles
- spongiotic blisters (inter epithelial cell edema)
- allergic contact dermatitis
- stasis dermatitis
- irritant dermatitis
- superficial fungal infections
- ballooning degeneration of epidermal cells
- acantholytic blisters
- basement membrane blisters
- other
Laboratory
- skin biopsy
- light microscopy
- direct immunofluorescence
- erythematous area adjacent to blister
- allows for identification of antibody deposition
- serology may be helpful with IgA pemphigus

Management
- varies with etiology
- systemic glucocorticoids
- other immunosuppressive agents
- successive or multiple agents are often needed
- treatment for months to years is usual
