Introduction
Tradename: Monopril.
Indications
- hypertension
- heart failure
- diabetic nephropathy
- nondiabetic proteinuric nephropathy
- scleroderma, scleroderma renal crisis
Dosage
Tabs: 10, 20 mg.
Pharmacokinetics
- fosinopril is about 99% bound to plasma proteins
- metabolized in the liver to an active metabolite
- elimination 1/2life is about 12 hours with normal renal function
- 50% of the drug is eliminated by the liver, 50% in the urine
- no dosage adjustments needed in patients with renal insufficiency

Adverse effects
- dry, non-productive cough
- rash
- taste disturbances
- worsening renal failure
- angioedema (rare)
- drug adverse effects of renin-angiotensin-aldosterone system inhibitors (RAAS inhibitors)
- drug adverse effects of ACE inhibitors
- drug adverse effects of antihypertensive agents
Drug interactions
- NSAIDs reduce may reduce hypotensive effect
- phenothiazines increase effects of ACE inhibitors
- allopurinol increases risk of hypersensitivity reactions
- lithium & digoxin levels are increased by fosinopril
- K+ salts & K+ sparing diuretics: increased risk of hyperkalemia
