Introduction
Tradename: Mycifradin.
Indications
- surgical prophylaxis for colorectal procedures (generally in combination with erythromycin)
- adjunctive agent in the treatment of hepatic encephalopathy
- otitis externa[4]
- skin infections[4]
Dosage
- hepatic encephalopathy:
- preoperative intestinal antisepsis
- adults:
- 1 g hourly for 4 doses, then 1 g every 4 hours for 5 doses, or 1 g at 1 PM, 2PM & 11 PM the day prior to surgery in combination with erythromycin
- children:
- 90 mg/kg/day divided every 4 hours for 2 days, or 25 mg/kg at 1 PM, 2 PM & 11 PM on the day prior to surgery in combination with erythromycin
- adults:
Tabs: 500 mg.
Solution: 125 mg/5 mL.
Pharmacokinetics
- only 3% of dose is absorbed from GI tract
- high concentrations achieved in the bowel & feces
- most is eliminated in the feces
- 1/2life is 2-3 hours
elimination via feces
elimination by hemodialysis = +
Adverse effects
- not common (1-10%)
- contact dermatitis, burning, erythema, rash, urticaria, contact conjunctivitis
- uncommon (< 1%)
Drug interactions
- neomycin may decrease absorption of warfarin
- drug interaction(s) anticonvulsants with anti-bacterial agents
- drug interaction(s) of antibiotics with warfarin
Mechanism of action
inhibition of bacterial protein synthesis
