neomycin (Mycifradin)

Introduction

Tradename: Mycifradin.

Indications

Dosage

  • hepatic encephalopathy:
    • adults: 4-12 g/day PO divided every 4 hours, or 500-2000 mg every 6-8 hours
    • pediatrics: 50-100 mg/kg/day divided every 4-6 h for 5-6 days, not to exceed 12 g/day
  • preoperative intestinal antisepsis
    • adults:
      • 1 g hourly for 4 doses, then 1 g every 4 hours for 5 doses, or 1 g at 1 PM, 2PM & 11 PM the day prior to surgery in combination with erythromycin
    • children:
      • 90 mg/kg/day divided every 4 hours for 2 days, or 25 mg/kg at 1 PM, 2 PM & 11 PM on the day prior to surgery in combination with erythromycin

Tabs: 500 mg.

Solution: 125 mg/5 mL.

Topical antibiotic.

Pharmacokinetics

  • only 3% of dose is absorbed from GI tract
  • high concentrations achieved in the bowel & feces
  • most is eliminated in the feces
  • 1/2life is 2-3 hours
