Indications
- evaluation of hemolytic anemia
Principle
- agglutination of erythrocytes by specific antihuman antibodies indicating that the erythrocytes are coated with human immunoglobulins or complement (C3d).
Clinical significance
Coomb's positive anemia
- pharmaceutical agents
- malignancies
- idiopathic
- positive DAT in 8-15% of hospitalized patients
- no cause found in 95% of these patients
- anti-IgG & anti-C3d
- anti-IgG only
- antibodies to Rh proteins
- drug-induced hemolysis
- anti-C3d only
- cold-agglutinin syndrome (Donath-Landsteiner Ab)
- most drug-related antibodies
- IgM antibodies
- IgG antibodies of low affinity
- activation of complement by immune complexes
