Introduction
Anemia with low erythrocyte mean corpuscular volume (MCV).
Etiology
- iron-deficiency anemia
- chronic blood loss
- iron-poor diet
- celiac disease
- anemia of chronic disease, chronic inflammation
- lead poisoning
- hyperthyroidism (occasional)
- genetic defects (some may be acquired)*
Genetics
- defects in SLC11A2 are a cause of hypochromic microcytic anemia
Laboratory
- complete blood count (CBC) with differential
- anemia: RBC count
- < 5 x 10E12/L in Fe deficiency
- > 5 x 10E12/L in thalassemia
- RDW
- > 16 in Fe deficiency
- < 16 in thalassemia
- microcytosis
- microcytic anemia in menstruating woman precludes need to prove iron deficiency provided normal prior blood hemoglobin (MKSAP19)
- anemia: RBC count
- reticulocyte count
- iron studies
- serum iron
- total iron-binding capacity (TIBC)
- serum ferritin
- transferrin saturation
- measured by serum iron/total-iron binding capacity (TIBC)
- <10% is consistent with iron deficiency
- >15% is consistent with anemia of chronic disease
- 10-15% indicates a bone marrow biopsy to distinguish iron deficiency from anemia of chronic disease
- peripheral smear
- fecal occult blood
- blood lead level
- low serum erythropoietin level confirms hypoplastic marrow
- hemoglobin electrophoresis
- target cells, sickle cells on peripheral smear
- splenomegaly
- evidence of bone remodelling
- hemoglobinopathy
- beta-thalassemia
- hemoglobin A2 3-7% in beta-thalassemia minor
- hemoglobin A2 7-90% in beta-thalassemia major
- free erythrocyte (zinc) protoporphyrin (FEP)
- increased in:
- normal in:
- bone marrow biopsy if indicated
Diagnostic procedures
- upper GI endoscopy
- colonoscopy for positive fecal occult blood

- treat specific etiology
