Introduction
A 4 page form consisting of 16 sections required by the Omnibus Budget Reconciliation Act (OBRA) for all nursing home residents within 14 days of admission, with selected sections to be updated quarterly.
The 16 sections include:

- activities of daily living (ADL)
- functional rehabilitation potential

- psychosocial well-being
