    • mitral stenosis
  • obstructive lung disease
    • COPD
    • obstructive sleep apnea
    • asthma
  • dyspepsia, pyrosis, GERD
  • bilateral diaphragmatic paralysis or paresis
• orthostatic hypotension
• mechanical ventilation
• increased intracranial pressure

Procedure

• generally 30 degrees of elevation
