Pathology
- stress & depression can provoke detrimental behaviors
Management
- cardiac rehabilitation & exercise can dimimish depression & improve fitness & function
- antidepressant may be indicated
- SSRI: sertraline & citalopram are 1st line
- bupropion, mirtazapine & trazodone: less evidence for safety & efficacy in cardiac patients
- SNRI may increase blood pressure & heart rate
- tricyclic antidepressants may worsen outcomes
- cognitive behavorial therapy in addition to antidepressant may be better than antidepressant alone
