Diagnostic criteria
- International Myeloma Working Group
- monoclonal plasma cells in the bone marrow > 10% &/or presence of biopsy-proven plasmacytoma
- monoclonal protein in the serum &/or urine
- myeloma-related organ dysfunction (1 or more)
- serum Ca+2 > 10.5 mg/dL (2.63 mmol/L)
- serum creatinine > 2 mg/dL (153 umol/L)
- hemoglobin < 10 g/dL
- lytic bone lesions or osteoporosis
- World Health Organization (WHO)
- major criteria
- plasmacytoma on tissue biopsy
- bone marrow clonal plasma cells > 30%
- high M protein on serum protein electrophoresis (IgG > 3.5 g/dL; IgA > 2.0 g/dL) or Bence-Jones protein in urine > 1.0 g/24 hours
- minor criteria
- bone marrow clonal plasma cells 10-30%
- M protein not reaching major criteria
- lytic bone lesions
- diminished normal immunoglobulins to < 50% of normal
- major criteria
