criteria for hospitalization of patients with pneumonia

Introduction
- elderly: age > 65 years
- significant comorbidity
- leukopenia (< 5000 WBC/mL) NOT attributable to known condition
- virulent pathogen suspected
- suppurative complications
- failure of outpatient management
- inability to take oral medication
- vital signs
- tachypnea (> 30 breaths/min)
- tachycardia (> 140 beats/min)
- hypotension (systolic BP < 90 mm Hg)
- hypoxemia (arterial pO2 < 60 mm Hg)
- alteration in mental status
- nursing home residents need not be hospitalized if:[2]
- able to eat & drink
- pulse < 100 minute
- respiratory rate < 30/min
- systolic blood pressure > 90 mm Hg
- paO2 > 92%, or > 90% with chronic lung disease
- change in status for the worse warrents hospitalization
* treatment with levofloxaxin 500 mg QD for 10 days
