Introduction
Prognostic index for COPD patients.
O airflow obstruction
D dyspnea
E exercise capacity
Derived from factors strongly associated with 1 year mortality
- lower body mass index
- lower FEV1 (airflow obstruction) (< 30% of predicted)[2]
- higher scores on a dyspnea scale
- shorter distance walked in 6 minutes (exercise capacity)
10 point scale: 0 (lowest risk) - 10 (highest risk)
Validation of index:
625 patients with COPD
52 month mortality
- 20% with scores 0-2
- 30% with scores 3-4
- 40% with scores 5-6
- 80% with scores 7-10
Predictors of mortality[2]
- FEV1 < 30% of predicted
- oxygen dependence
- multiple hospitalizations for COPD exacerbation
- significant comorbidities
- weight loss & cachexia
- diminished functional status
- increasing dependence on others
