guidelines for timing initial screening for lung transplantation

Introduction

Screening:

• chronic obstructive lung disease
  • FEV1 < 30% predicted, postbronchodilator
  • resting hypoxia (paO2 < 55-60 mm Hg)
  • hypercapnia (paCO2 > 50 mm Hg)
  • significant pulmonary hypertension (secondary)
• cystic fibrosis
  • FEV1 < 30% predicted, postbronchodilator
  • resting hypoxia (paO2 < 55-60 mm Hg)
  • hypercapnia (paCO2 > 50 mm Hg)
• idiopathic pulmonary fibrosis
  • vital capacity or total lung capacity < 60% of predicted
  • resting hypoxia
  • significant pulmonary hypertension (secondary)
• primary pulmonary hypertension
  • New York Heart Association class 3 or 4 heart failure
  • mean right atrial pressure > 10 mm Hg
  • mean pulmonary arterial pressure > 50 mm Hg
  • cardiac index < 2.5 L/min/m2
