Introduction
Screening:
- chronic obstructive lung disease
- FEV1 < 30% predicted, postbronchodilator
- resting hypoxia (paO2 < 55-60 mm Hg)
- hypercapnia (paCO2 > 50 mm Hg)
- significant pulmonary hypertension (secondary)
- cystic fibrosis
- idiopathic pulmonary fibrosis
- vital capacity or total lung capacity < 60% of predicted
- resting hypoxia
- significant pulmonary hypertension (secondary)
- primary pulmonary hypertension
- New York Heart Association class 3 or 4 heart failure
- mean right atrial pressure > 10 mm Hg
- mean pulmonary arterial pressure > 50 mm Hg
- cardiac index < 2.5 L/min/m2
