Etiology

• primary hypoalphalipoproteinemia
• secondary causes of diminished HDL:
  • type 2 diabetes mellitus
  • impaired glucose tolerance
  • insulin-resistance
  • obesity
  • androgen excess in females
  • renal failure
  • pharmaceutical agents
    • retinoids
    • beta-blockers
    • anabolic steroids*

* selective reduction in HDL; other causes of diminished HDL also elevate serum triglycerides

Management

• nicotinic acid (niacin) is the drug of choice
