Introduction
Guidelines:
- symptoms of recurrent congestive heart failure at rest
- New York Heart association class IV
- ejection fraction < 20%
- optimal treatment with diuretics & vasodilators
- loop diuretic or ethacrynic acid
- vasodilator
- nitrate plus hydralazine is acceptable
- ACE inhibitor is preferable
- symptomatic supraventricular or ventricular arrhythmias resistant to antiarrhythmic therapy
- history of cardiac arrest
- history of unexplained syncope
- embolic CVA of cardiac origin
- concurrent HIV disease
