Indications
- drug-resistant epilepsy
- refractory depression (not particulary effective)
- prevention of cluster headaches[5]
Contraindications
- implantation of the device becomes a contraindication for magnetic resonance imaging (MRI), excepting MRI of the head in special circumstances[4]
Procedure
- surgical implantation of stimulator in upper chest
- generally performed under general anesthesia
- provides programmed pulsed electrical stimulation to vagus nerve in neck
- in addition to programmed stimulation, patients are given a magnetic device that can be used to trigger on-demand stimulation
* SenTiva FDA-approved device
* gammaCore vagus nerve stimulator (FDA-approved for cluster headaches)[5]
Mechanism of action
- unclear
- 50% reduction of seizure activity within 1 year of implantation[4]
Complications
- generally well tolerated
- hoarseness
- throat pain
- cough
- dyspnea
