Indications

• evaluation of rapidly progressive dementia for treatable causes

Procedure

• 1 point each for
  • seizures
  • disease associated tumor
  • age < 50 years at onset
  • brain MRI suggestive of autoimmune encephalitis
  • mania
  • movement disorder
  • CSF pleocytosis (>= 10 cells/uL)

Interpretation

• score of 1 at presentation captures 95% of potentially treatment-responsive cases
• all patients with score of >= 3 have potentially treatment-responsive case (sensitivity 26%, specificity 100%)

Notes

• false-negatives may include
  • Wernicke-Korsakoff syndrome
  • small-vessel vasculitis
  • LGI1 encephalitis
  • cerebral amyloid angiopathy with inflammation
