major depression with psychosis; psychotic/delusional depression

Etiology

• may be manifestation of major depression following stroke
• may occur in patients with illness anxiety disorder

Clinical manifestations

• combination of depression & psychosis
  • psychosis often manifests as belief bad things are going to happen

Management

• treatment with both antidepressant & antipsychotic if early in the course of disease
• hospitalize for electroconvulsive therapy if resistant depression, or urgency due to rapid deterioration (substantial weight loss, frailty, agitation)
