Polatuzumab Vedotin (POLIVY)

Indications

• component of rituximab, cyclophosphamide, doxorubicin, Polatuzumab Vedotin, & prednisone (pola-R-CHP) used for treatment of previously untreated intermediate- risk or high-risk diffuse large B-cell lymphoma (DLBCL)
• indicated in combination with bendamustine & a rituximab product for treatment of adults with relapsed or refractory diffuse large B-cell lymphoma, after at least two prior therapies

Dosage

• 1.8 mg/kg as an intravenous infusion over 90 minutes every 21 days for 6 cycles in combination with bendamustine & a rituximab product
  • subsequent infusions may beadministered over 30 minutes if the previous infusion is tolerated
• six cycles of either pola-R-CHP or R-CHOP, plus two cycles of rituximab alone

* Premedicate with an antihistamine & antipyretic

Injection: 30 mg or 140 mg of polatuzumab vedotin-piiq as a lyophilized powder in a single-dose vial.

Adverse effects

• peripheral neuropathy
• myelosuppression
• progressive multifocal leukoencephalopathy
• hepatotoxicity
• tumor lysis syndrome

• drug adverse effects of pharmaceutical monoclonal antibodies

Mechanism of action

• antibody-drug conjugate targeting CD79b, ubiquitously expressed on surface of malignant B cells
