urinoma

Etiology

• blunt trauma
  • extravasation of urine is common with severe blunt trauma
  • spontaneous occurs in most cases
  • urinoma formation occurs uncommonly
• ureteral obstruction due to
  • cancer
  • urinary calculus
  • pregnancy
  • congenital anomaly

Pathology

• urine collection in perirenal fat as a result of leakage from the renal pelvis of renal calices
• an inflammatory response with lipolysis results in fibrous encapsulation

Clinical manifestations

• gradual progression
• pain & pressure depend on size & location

Laboratory

• renal function tests

Diagnostic procedures

• image-guided percutaneous needle aspiration is both diagnostic & therapeutic

Radiology

• renal ultrasound
• computed tomography
• intravenous pyelogram

Complications

• abscess formation
• peritonitis
• sepsis
• fibrosis
• granuloma formation

Management

• drainage with CT or ultrasound guidance
• treatment of urinary obstruction
