Introduction
- multicomponent strategy to prevent functional decline & cognitive decline in hospitalized elderly[4]
- patients, caregivers, providers, & HELP staff work together to address
Clinical significance
- reduces frailty at hospital discharge[1]
- reduces in hospital delirium[4]
- reduces in hospital falls[4]
- reduces health care costs[4]
- benefit diminished 3 months after discharge[1]
- trends toward decreasing length of stay & placement in a nursing home have not been definitely shown[4]
Procedure
- reduces incidence of delirium & in-hospital falls
- includes 3 postoperative nursing interventions
- early mobilization
- oral & nutritional assistance
- orienting communication
