Pathology
- depressed patients with Parkinson's disease & patients with major depression have similar regional cerebral blood flow, assessed by SPECT
- frontal-limbic networks are involved
- non-depressed patients with Parkinson's disease have lesser activity in the right frontal cortex
- 1.1% of patients with depression will develop Parkinson's disease (PD) vs 0.4% of patients without depression[4]
- depression may be a very early symptom of PD or a risk factor[4]
Management
- antidepressants, SSRI, SNRI
- paroxetine (Paxil) & venlafaxine (Effexor) effective without adversely affecting motor function[3]
- avoid antidepressants with anticholinergic activity (tricyclic antidepressants)
- repetitive transcranial magnetic stimulation
