Introduction

Example:

I was called to the bedside of the patient to pronounce him/her dead. He/she was motionless without visible or palpable respirations and unresponsive to voice, physical or painful stimuli. The precordium was quiet. Pupils were fixed and dilated. Corneal reflex was absent.

The patient is pronounced dead on xx/yy/zz at xx am/pm.

After pronouncing the patient dead, contact the patient's family or next of kin. Tell them that the patient died peacefully & did not suffer (unless this is not true). Invite them to come see the patient before he/she is taken to the morgue. Request an autopsy.
