alarm features of dyspepsia

Clinical manifestations

• age > 55 years with new-onset symptoms
• family history of proximal gastrointestinal cancer
• unintentional weight loss
• gastrointestinal bleeding
• history of peptic ulcer
• progressive dysphagia, odynophagia
• unexplained iron-deficiency anemia
• persistent vomiting
• palpable mass or lymphadenopathy
• gastric surgery
• jaundice (not mentioned in)
• diaphoresis, unexplained fatigue, nausea, vomiting, exertional symptoms suggest angina pectoris masquerading as dyspepsia

Diagnostic procedures

• upper gastrointestinal endoscopy
• electrocardiogram if any doubt this could be angina pectoris*

* atypical presentations of cardiac ischemia & myocardial infarction are more common in women, older adults, & patients with diabetes mellitus
