nystatin (Mycostatin, Nilstat, Nystop, Bio-statin)

Introduction

Tradename: Mycostatin, Nilstat.

Indications

• treatment of non-invasive, generally topical fungal infections
• most active against Candidal yeast infections

Dosage

• Oral agent: 4-6 mL PO swish + swallow QID.
• Infants: 2 mL/dose, with 1 mL in each side of mouth.

Solution: 100,000 units/mL Suspension (oral) 100,000 units/gram (60 mL). Tablet (oral): 500,000 units

• Topical agent: Candidiasis: apply BID/TID

Cream: 100,00 units/gram (15 g, 30 g)

Ointment: 100,000 units/gram (15 g, 30 g)

Powder: (topical) 100,000 units/gram (15 g)

• Vaginal infections: 1 tab per vagina QD or BID

Vaginal tablet: 100,000 units (15 & 30 boc with applicator)

Troches: 200,000 units

Pharmacokinetics

not absorbed orally or topically

elimination via feces

Adverse effects

• not common (1-10%)
  • nausea/vomiting, diarrhea, stomach pain
• uncommon (< 1%)
  • hypersensitivity reactions

Mechanism of action

• antifungal/antibiotic: 3 active components; A1, A2 & A3
• obtained from Streptomyces
• binds to membrane sterols resulting in increased permeability
• fungistatic & fungicidal
