Introduction

Etiolohy:

• anti-glomerular basement membrane [GBM] disease
• Goodpasture's syndrome
• tubulointerstitial nephritis

Pathology

• the antigen is an epitope from type 4 collagen found in alveolar & glomerular basement membranes
• > 50% have lung involvement with hemoptysis, shortness of breath. or cough

Laboratory

• glomerular basement membrane Ab in serum

Management

• plasmapheresis
• pulse glucocorticoid followed by oral prednisone
• cyclophosphamide
