remitting seronegative synovitis with pitting edema

Etiology

• part of the clinical spectrum of polymyalgia rheumatica
• associated with
  • autoimmune disease
  • neurodegenerative disease: Parkinson's disease
  • paraneoplastic syndrome

Epidemiology

• elderly patients, uncommon

Genetics

association with HLA-B27

Clinical manifestations

• pain, swelling, stiffness & numbness in the hands
  • inflammatory synovitis of the wrists & palmar tendon sheaths
  • both flexor & extensor tendons affected
• can affect both hands & feet
• stiffness of the shoulders
• symptoms worse upon awakening, except for numbness
• numbness occurs more often at night
• no evidence of Raynaud's phenomenon

Management

• NSAIDs
• glucocorticoids, treat as polymyalgia rheumatica
