Introduction
- LV ejection fraction > 30%
- angina refractory to optimal medical therapy
- reversible ischemia of the left ventricular free wall & coronary artery disease corresponding to region of myocardial ischemia
- not be amenable to coronary artery bypass graft (CABG) or percutaneous transluminal coronary angioplasty (PTCA)
- angina either due to
- severe diffuse disease
- lack of suitable targets for complete revascularization
- lack of suitable conduits for complete revascularization
