Introduction
An EMR-based set of age-specific dose and frequency defaults for patients >= 75 years.
Indications
- prevent drug overdose in hospitalized elderly
Clinical significance
- significant dose decreases at 1 year for 9 of 10 most commonly used medications
- 8 of 10 medications maintained the initial dose decreases after 42 moonths
- 8 of 10 medications were prescribed at more age-friendly doses & frequencies compared to baseline after 42 months
