Indications
- hypercoagulability
- prophylaxis for venous thromboembolism
- atrial fibrillation
- prevention of embolic stroke
- treatment of deep vein thrombosis (DVT)
- acute myocardial infarction (AMI)
- transient ischemic attacks (TIA)
- prosthetic heart valves
* including during pregnancy if the dose is 5 mg QD or less, otherwise LMW heparin
