hemosiderin in urine

Indications

• evaluation of recent episode of hemoglobinuria
• evaluation of renal involvement in hemochromatosis

Reference interval

• < 0.1 mg/day

Principle

• free hemoglobin is rapidly filtered by the renal glomeruli & reabsorbed in the proximal tubules where it is catabolized into ferritin & hemosiderin
• hemosiderin will appear in the urine 2-3 days after an episode of acute hemolysis that resulted in hemoglobinuria
• at this time hemoglobin in urine is often negative

• renal involvement in hemochromatoisis
• chronic hemolysis
  • prosthetic heart valves
