trifluridine (Viroptic)

Introduction

Tradename: Viroptic.

Indications

• treatment of primary keratoconjunctivitis & recurrent epithelial keratitis caused by Herpes simplex virus types 1 & 2
• effective in treatment of epithelial keratitis resistant to idoxuridine

Dosage

• 1 drop every 2 hours while awake, up to 9 drops/day
• continue with 1 drop every 4 hours for an additional 7 days after re-epithelialization of corneal ulcer
• do NOT exceed 21 days of therapy

Solution: 1%. (7.5 mL).

Adverse effects

• not common (1-10%)
  • burning, stinging
• uncommon (< 1%)
  • palpebral edema, epithelial keratopathy, keratitis, stromal edema, increased intraocular pressure, hyperemia, hypersensitivity

Mechanism of action

• replaces thymidine in viral DNA synthesis
• causes production of defective viral proteins
• reversibly inhibits thymidylate synthetase
