sodium thiosulfate (Sodothiol)

Indications

• cyanide poisoning
  • used in combination with sodium nitrite in the cyanide antidote pack
• arsenic poisoning

Dosage

• cyanide & nitroprusside (poisoning)
  • 2.5-5 mL/min IV to a maximum of 50 mL
  • see cyanide antidote pack
• arsenic poisoning
  • 1 mL on 1st day
  • 2 mL on 2nd day
  • 3 mL on 3rd day
  • 4 mL on 4th day
  • 5 mL on alternate days, thereafter

Injection: 100 mg/mL (10 mL)

Pharmacokinetics

• distributes to extracellular fluid
• 1/2life 30-40 minutes
• excreted unchanged in the urine

Adverse effects

• not common (1-10%)
  • hypotension, coma, CNS depression (secondary to thiocyanate toxicity), psychosis, tinnitus, confusion, weakness, contact dermatitis, local irritation

Poisoning: Contact Regional Poison Control Center as needed for consultation on use as an antidote
