Introduction
Alloantibody to factor VIII occurs in 20-25% of patients with severe hemophilia A transfused with factor VIII.
Laboratory
- prolonged aPTT, NOT corrected with addition of normal plasma
- coagulation factor VIII inhibitor in plasma
Management
- immunosuppressive therapy
- acute bleeding
- porcine factor VIII (generally less reactive with anti-human factor VIII)
- prothrombin complex concentrate
- recombinant factor VIIa concentrate
- plasmapheresis
