A rating scale based on 5 criteria. (1961)
- physical condition
- very bad: 1
- poor: 2
- fair: 3
- good: 4
- mental condition
- activity
- bedbound: 1
- chairbound: 2
- walk/help: 3
- ambulatory: 4
- mobility
- immobile: 1
- very limited: 2
- slightly limited: 3
- full: 4
- incontinent
- doubly (fecal incontinence & urinary incontinence): 1
- usually incontinent of urine: 2
- occasional: 3
- not incontinent: 4
A total score of < 12-15 indicates a high risk.
