Model characteristics:

• Medicare risk-based managed care program for long-term NH residents
• nurse practitioners are employees of EverCare
• MDs are paid by EverCare on a fee-for-service basis
• NPs provide most of the direct care

Assumptions of the model:

• more aggressive primary care management will reduced acute hospitalization & costs
• increased communication with families will increase satisfaction
• increased interaction with nursing & other providers will result in higher quality care
