- vital signs
- rapid response team with specific activation criteria
- team leaders typically are critical care nurses & respiratory therapists; some institutions include physicians or advanced care practitioners (nurse practitioners or physician assistants)
- at least one clinician member should have expertise in eliciting patients' goals of care & establishing treatment plans that reflect patients' wishes & prognosis
- patients, families, & care partners should be empowered to alert hospital personnel & the rapid response team
