Medicare GUIDE dementia care model

Introduction

• pilot program will test a per-patient per-month amount payment model for people who provide support services to patients with dementia
• Medicare Part B-enrolled providers able to bill for Medicare Physician Fee Schedule services & agree to requirements of the GUIDE model are eligible to apply

Procedure

• participating providers will establish dementia care programs
  • people with dementia & their caregivers will be assigned 'care navigators' who will help them access services & support
    • services & support include
    • personalized assessments
    • care plans for patients
    • 24/7 access to a support line
