Introduction

Spine pain is defined as pain that occurs in the neck or back.

Management

• 6-8 weeks of an individualized postural therapy* or a multidisciplinary biopsychosocial intervention known the Identify, Coordinate, & Enhance (ICE) care model that includes physical therapy are associated with small reductions in pain-related disability at 3 months vs usual care
• effects are sustained & clinically meaningful at 12 months
• spending did not differ significantly between ICE & usual care
• individualized postural therapy increased spending vs usual care
• for chronic axial spine pain
  • recommendation against epidural, facet-joint, sacroiliac, or paravertebral muscle injections of local anesthetics, steroids, or both
  • recommendation against facet-joint or sacroiliac radiofrequency ablation
• for chronic radicular pain
  • recommendation against epidural injections of local anesthetics, steroids, or both
  • recommendation against radiofrequency stimulation of dorsal root ganglia
