Pathology

• chronic renal failure
• dysregulation of calcium & phosphorus homeostasis
  • alterations in plasma calcium, plasma phosphorus & regulatory hormones (PTH)
  • alterations in bone mineralization (osteopenia)
• dystrophic calcification

Laboratory

• serum phosphorus elevated
• serum calcium may be elevated

Management

• normalize plasma phosphorus
  • non-calcium based phosphate binders
    • sevelamer, lanthanum carbonate
• avoid hypercalcemia
