biliary cast syndrome

Etiology

• liver transplantation for primary biliary cirrhosis

Pathology

• dilatation of the lower end of the common bile duct
• filling defect at the upper end of the common bile duct

Laboratory

• bilirubin in serum -> marked hyperbilirubinemia
• mildly elevated serum ALT, serum AST (< 100 U/L)

Radiology

• magnetic resonance cholangiopancreatography: thinning of intrahepatic bile duct

Management

• surgery & a 2nd liver tranplantion may be necessary
