Involuntary confinement of a person in a locked room. It is the final step in keeping a patient from harming self or others

Indications

• may only be used for behavioral health reasons

The nurse is responsible for no more than 2 patients

Documentation every 15 minutes

At least one staff member required to accompany the patient off the unit (not discharge)
