Created by Centers for Medicare & Medicaid (CMS) to facilitate coordination of care, quality of care & cost-effectiveness.

Different models vary by what is bundled in the payment, what DRGs are included, whether provider services are bundled, & whether payment in prospective or retrospective.

In model 3, bundled payment is triggered by hospitalization. The organization receiving payment determines payment for each provider & facility during the entire episode of care.
