Notes
- allows patient to remain with primary care provider
- provides proactive, comprehensive health care by physician- nurse teams for patients with multiple chronic conditions with frequent & ineffective encounters with the healthcare system
- each team consists of a registered nurse working with 2-5 physicians & other staff
- team assesses patient & caregiver at home, creates care plan, monitors patient's status monthly, promotes self care, coordinates provider care, provides transitional care (hospital to home), educates & supports family & caregivers, facilitates access to community resources
