Introduction

Over the last 2 weeks, how often have you been bothered by any of the following problems?

Select & score each of the 4 items from the following:

0 = Not at all

1 = Several days

2 = More than half the days

3 = Nearly every day

• Little interest or pleasure in doing things
• Feeling down, depressed, or hopeless
• Not being able to stop or control worrying
• Feeling nervous, anxious or on edge

Questions 1 & 2 constitute an depression subscale, questions 3 & 4 a anxiety subscale

Scores on each subscale can range from 0-6

A score of 3 or greater on a subscale warrants further evaluation.
