  • mild systemic disease = 2
  • severe systemic disease = 4
  • severe systemic disease with constant threat to life = 5
  • moribund patient; not expected to survive without surgery = 6
• surgery-specific risk
  • mastectomy, joint replacement, appendectomy, hysterectomy or other low-risk surgery = 0
  • infra-inguinal vascular surgery, Nissen fludoplasty or other intermediate-risk surgery = 1
  • abdominal aortic aneurysm repair, liver resection, pancreatic resection or other high-risk surgery = 2
• urgency of surgery
  • elective or non-emergent = 0
  • emergency surgery = 1
